Learning objectives: The aim of the present work is to report our experience in the therapeutic management of utero-ovarian "risky" anastomoses during UAE procedures in patients undergoing endovascular treatment of uterine fibroids.

Conclusion: UAE treatment offers a minimally invasive alternative to surgery even in the presence of anatomic variants; with proper endovascular procedural strategies is possible to preserve the vascularization of adnexal structures.
