Purpose: The Gleason Score (GS) is an important factor to define biological activity and aggressiveness of prostate cancer, it influences the choice of treatment and it is an important predictor of treatment response. Methods and materials: Between February and December 2015, we evaluated retrospectively 127 patients (age 45-75 years), underwent to mpMRI 3T with both pelvic phased array 32 channel coil and an endorectal coil for suspicious CaP. Results: An inverse correlation between ADC and GS resulted. Malignant lesions had ADCmin e ADCmed values included between 500-1000 x10-6 mm2/sec. 46 (61,3%) patients had ADCmed  and ADCmin included between 800-1000 x10-6mm2/sec; 44 of them (95,6%) had a histological GS between 5 and 7(3+4), and 2 of t[...]

Conclusion: Our experience confirmed, according to international literature, an inverse correlation between ADC and GS. Therefore, ADC value shows the potential usefulness for non-invasive characterization of prostate cancer aggressiveness and to improve the therapeutic planning.
